Estimated Hospital Privileging Timeframe- 30-60 days MD/DO - Education/Training Requirements One or more years of training, at least 3-year resident in approved EM/FM/IM or completion of ACGME/AOA approved residency program MD/DO - Clinical Experience Requirements 3rd Year Residents accepted (Moonlighting privileges only approved until June 30th upon residency completion, then new app is needed through CVO); No Locums Accepted MD/DO - License Requirements Active unrestricted LA Medical License, LA State Controlled Substance, DEA w/ LA address & schedules 2,2N,3,3N,4 & 5 and Supervising Physician License with Louisiana State Board of Medical Examiners MD/DO - Board Certification Requirements Board Certified/Eligible in EM/FM/IM preferred MD/DO - Merit Badge Requirements ACLS, ATLS, PALS required if not EM trained MD/DO - Immunization Requirements Flu (within current flu season | TB (past 12 months) or Chest X-Ray with positive TB Vaccinations MD/DO - Case Log Requirements Documentation of case logs for past 2 years. MD/DO - CME/CEU Requirements Proof of completed CME's for the last two years.
