RN Clinical Nurse Coordinator

Join a collaborative pulmonary team in a weekday, outpatient role with no on-call requirements. This position offers a healthy work-life balance with MondayFriday hours and the opportunity to work from home two days per week after six months. Based primarily at Northwest Tower, the role supports care across multiple clinic locations while partnering closely with pulmonary physicians. In this position, the RN Clinical Nurse Coordinator coordinates patient care, follows up on test results, manages both phone and in-clinic patient interactions, and assists with ordering tests and equipmentall within EPIC.

Primary Duties & Responsibilities:

• Provides direct patient care, including assessment, planning, implementation and evaluation of patients; completes health history and physical examination on patients; conveys findings to physician.
• Coordinates patient care, orders diagnostic tests, conveys findings to physician, attends inpatient rounds, coordinates patient appointments and assists with referrals to outside sources.
• Assists physician and provides support with examinations, procedures and other processes related to direct patient care.
• Performs a variety of nursing administrative duties, including compliance with environmental safety regulations, infection control standards and quality assurance.
• Acts as liaison and team leader by facilitating communication and coordination of care between patients, families and health team members; explains routines and procedures to patients and coordinates patient care information to appropriate sources.

Salary Range: $60,800.00 - $105,700.00 / Annually
