About the Role:

The Director of Social Services leads the facility’s social services function and ensures that each resident’s psychosocial needs, rights, preferences, and goals are identified and addressed throughout the resident’s stay. This role works closely with residents, families, responsible parties, and the interdisciplinary care team to support person-centered care, safe transitions, resident advocacy, and quality outcomes.

Minimum Qualifications:

• Bachelor’s degree in Social Work, Public Administration, Human Services, or a related field.
• Must be licensed - LCSW or LMSW
• At least 7 years of progressive experience in social services, including 3 years in a supervisory or management role.
• Strong knowledge of social service programs, policies, and regulatory requirements in the United States.
• Demonstrated experience in program development, staff management, and budget oversight.
• Excellent communication, leadership, and interpersonal skills.
• Working knowledge of EMRs, specifically PCC.

Responsibilities:

• • Direct, organize, and evaluate the facility’s social services program in accordance with resident needs, facility policies, and applicable federal, state, and local requirements.
  • Complete or oversee timely psychosocial assessments, care-plan documentation, progress notes, discharge planning records, and other required documentation.
  • Participate actively in the interdisciplinary care-planning process and ensure that resident preferences, cultural considerations, behavioral health needs, support systems, and discharge goals are reflected in the plan of care.
  • Advocate for resident rights, dignity, choice, privacy, self-determination, and access to services; promptly escalate concerns involving abuse, neglect, exploitation, discrimination, or potential rights violations.
  • Facilitate resident and family meetings, care conferences, and conflict-resolution discussions using clear, respectful, and trauma-informed communication.
  • Respond to grievances and complaints, document follow-up, identify trends, and collaborate with leadership on corrective and preventive actions.
  • Support advance care planning and end-of-life preferences in collaboration with residents, responsible parties, nursing, medical providers, hospice, and other appropriate disciplines.
  • Recruit, orient, train, schedule, coach, and evaluate social services staff, when applicable; promote appropriate caseload management and professional development.
  • Participate in quality assurance and performance improvement activities, facility assessments, audits, surveys, investigations, and plans of correction.
  • Maintain confidentiality and safeguard resident information in accordance with privacy requirements and facility policy.

Skills:

The Director of Social Services utilizes strong leadership and organizational skills daily to interact with all stakeholders. Communication skills are critical for engaging with staff, residents, family members, etc..
