Job Summary:
We are seeking a compassionate and caring Nursing Assistant to join our team at SYNERGY HomeCare of East Haven in Madison, Connecticut. As a Nursing Assistant, you will play a vital role in providing high-quality care to our clients, enhancing their quality of life, and promoting independence. Responsibilities:
Assist clients with daily living activities, such as bathing, dressing, and grooming, in a safe and respectful manner
Provide personal care, including toileting, transferring, and ambulation assistance as needed
Monitor and report changes in clients' condition to licensed nursing staff
Assist with medication reminder.
Develop and maintain positive relationships with clients and their families, fostering a supportive and inclusive environment
Collaborate with interdisciplinary teams to develop and implement individualized care plans, promoting effective communication and coordination of care

Requirements:
1 Year experience
Drive's License
High school diploma or equivalent required
Ability to lift 25 pounds or more
