If you want to make an impact by helping people live their best life, read the details below.Provides positive support and be an advocate for the warriorProvides assistance and guidance with identifying choices and making informed decisions that involve warrior's activities and lifeProvides structure and support in household management such as: maintaining cleanliness of home environment and establishing a safe, healthy and efficient routineAssists with participation in recreation and community activities in order to promote socialization and establish relationships with peersAssists warrior with improving community living and integration skills in a variety of settings such as shopping, church attendance, sporting events, club participation, classroom support, volunteer/employment supportWorks with warriors to learn how to participate in their desired recreational and social activities within their surrounding communityProvides training or assistance with mobility in home environment, using adaptive aids, and accessing public transportationProvides assistance with developing effective and appropriate communication and social skillsOperates the program within organizational guidelines, policies, professional and ethical standardsMaintains organization confidentiality of information relating to individuals and their families in accordance with Health Insurance Portability and Accountability Act/company policiesTreats clients, families, co-workers, and outside parties with respect and dignity, honoring all values and all written and reasonably implied ethical practices of the organizationBased on the established goals of an individualized Care Plan provides warriors functional supports to acquire, retain and improve independenceHelps warriors with reintegrating and reconnecting with their communities in order to encourage a healthy level of independenceIdentifies barriers, triggers, or changes in functional status in order to develop proactive interventions that promote continued independenceEducates warriors on how to develop necessary problem solving skills and strategiesAccompanies warriors to locations and appointments in the community, to educate them in learning how to assess and utilize their community resources and servicesServes as a role/mentor for warriors and maintains good communication/relationships with warrior, their families, and referral sourcesCompletes daily progress notes and other necessary documentation in compliance with organization policies and provides to own agency and to NCC Case ManagerResponsible for monitoring and appropriately utilizing billable hours with warriorsConducts professional behavior at all timesParticipates in training at their agency and necessary and appropriate training through National Certified CounselorCommunicates regularly with organization personnel regarding warrior issues/concernsParticipates in team meetings as requestedPerforms other duties as assigned to meet the program goals or as requested by National Certified Counselor and own agencyOther duties as assignedBachelor's Degree in related field and experience listed below OR Associates Degree with one year experience in related field and experience listed below OR High School Degree/Equivalency with three years experience in related field and experience listed belowDemonstrates knowledge of brain injury, accessibility to community resources and experience with appropriate training skills, assessment and evaluation skillsCertified Brain Injury Specialist (CBIS) preferredMust have a valid driver's license and the vehicle is compliant with local Department of Motor Vehicle standards such as current registration, inspection, and insuranceMust pass criminal background check, motor vehicle checks and a sex offender registry checkAll Ways Caring HomeCare delivers quality, compassionate, and individualized care and support that helps people in need of assistance stay at home – all while maximizing their dignity, privacy, and independence.
