Requirements:

ABOS Board Certified or TRULY Board Eligible in Orthopedic Sports Medicine (must become certified within 5 years) - REQUIRED no exceptions
Valid IL & IA license or Active IMLC Willing to Obtain Licenses - REQUIRED
Strong arthroscopic and open sports surgery skills REQUIRED , shoulder, knee, hip PREFERRED
Hold current and unrestricted state and federal authority to prescribe and administer medications as necessary or appropriate to the field of practice - REQUIRED
DEA - REQUIRED
BLS Certification and maintain recertification - REQUIRED
Clean malpractice and license history - HIGHLY PREFERRED / PREFERRED
Does the provider require visa support - No J-1 or H-1B (Note in Additional Submission Details) - REQUIRED
Submit Self-Query NPDB pulled within 30 days from submission at time of presentation - REQUIRED
