LTSS Field Care Manager - Travel Required

Molina Healthcare is seeking a Field Care Manager for LTSS to conduct in-home assessments, coordinate waiver services, and develop care plans for Medicaid members in the Lancaster, TX area. This role requires travel and strong LTSS/case management experience. Responsibilities include coordinating with caregivers, physicians, and the health care team; monitoring progress; and ensuring timely documentation.
