LTSS Care Manager — Home-Visit Waiver Specialist
Molina Healthcare of Illinois
Molina Healthcare is seeking a TX-licensed LVN/LBSW/LMSW to join our LTSS Team as a Case Manager serving Medicaid members in southern Dallas County. The role includes face-to-face home visits and completing assessments to determine services, with mileage reimbursed. Preferred candidates have LTSS experience. This full-time position follows a Monday–Friday schedule, 8 AM–5 PM CST, with travel 25-40% as required by state contracts. #J-18808-Ljbffr Molina Healthcare
