About the Role:

As an LPN at Woodland Grove Health and Rehab, you will be responsible for providing high-quality, patient-centered care to our residents. Your duties will include:

• Administering medications and treatments as prescribed by physicians

• Monitoring vital signs and reporting changes to medical staff

• Developing and implementing individualized care plans in collaboration with the interdisciplinary team

• Providing support with activities of daily living (ADLs) such as bathing, dressing, and toileting

• Educating residents and families on care and treatment plans

Responsibilities:

• Provide direct patient care to residents, including administering medications, performing vital signs, and assisting with ADLs

• Collaborate with the interdisciplinary team to develop and implement individualized care plans

• Maintain accurate and detailed records of resident care and treatment

• Participate in quality improvement initiatives to ensure excellence in patient care

• Maintain a clean and organized environment, adhering to infection control and safety standards

Requirements:

• Current LPN licensure in the state of Florida

• High school diploma or equivalent required
