Coord Stroke Program-Telestroke Program Full Time Days

Description The Stroke Program Coordinator is an experienced clinician who oversees the care of stroke patients. This key team member performs duties in support of and in compliance with The Joint Commission, other licensing/accreditation bodies, and performance improvement plans. Responsibilities include data collection, analysis, continuous quality improvement, patient satisfaction initiatives, and community outreach, as well as patient guidance, education, and follow‑up. The role works collaboratively with staff and physicians to create seamless coordination of care for the stroke patient population. Qualifications Required Qualifications Minimum of 5 years RN experience BSN Current State of Illinois RN License NIHSS certification Previous stroke experience Proficient in Microsoft Word, Excel, PowerPoint Preferred Qualifications Familiarity with TJC DSC for stroke
