The salary range for this position is $35.64 - $49.89 (Hourly Rate). Placement within the salary range is dependent on relevant work experience and internal equity. For positions represented by a labor union, placement is guided by the collective bargaining agreement. Role Overview Position: Social Worker-LCSW, Hispanic Transplant Program (Full Time, Days) Language: Bilingual – English/Spanish. In this role, the Hispanic Transplant Program Social Worker will provide both patient/clinical care and dialysis unit outreach and education. The responsibilities include Performing biopsychosocial assessments for abdominal transplant patients and families. Coordinating care plans and collaborating with the interdisciplinary team. Providing supportive counseling to patients and families before and after transplant. Assisting patients with navigating complex financial and insurance issues. Referencing outside mental health providers and agencies as needed. Advocating for patients to access necessary information, resources and services. Educating patients and dialysis unit staff about kidney transplant options and materials. Completing timely documentation of all activities in the medical record and hospital‑wide information systems. Engaging in quality improvement activities as required. Additional Responsibilities Coordinating plans of action when barriers exist to facilitate resolution. Collaborating and communicating with the interdisciplinary team throughout all phases of planning. Seeking consultation with and making referrals to appropriate disciplines/departments. Acting as an advocate for patients who require assistance to gain access to information, resources and services. Managing and informing members of the healthcare team regarding necessary interventions. Providing patients and families with education to promote wellness and increase knowledge of the healthcare system. Completing timely documentation of all activities in the medical record and hospital‑wide information systems. Engaging in quality improvement, as required. Qualifications Licensure in Illinois; Licensed Clinical Social Worker (LCSW). Master's Degree in Social Work from a CSWE‑accredited school. Minimum of two years post‑graduate experience in hospital social work or related settings. High level of interpersonal skills to affect positive outcomes. Organizational skills to prioritize and manage a caseload while performing social worker functions; self‑direction required. Analytical skills to independently collect, analyze, and interpret data; resolve problems requiring innovative solutions and negotiate sensitive situations.
