Key Responsibilities\n

• Conduct thorough assessments of clients' counseling needs, focusing on mental health and substance abuse. Utilize diagnostic tools such as the DSM (Diagnostic and Statistical Manual) to diagnose clients accurately based on their unique situations.

• Prepare comprehensive treatment plans for clients with a focus on substance use disorders, employing criteria established by ASAM (American Society of Addiction Medicine).

• Collaborate with clients to develop individualized discharge and continuum of care plans to support their transition to appropriate levels of care.
\n
• Document all therapy sessions and assessments diligently in the client’s clinical record, ensuring compliance with ethical and legal standards.
\n
• Manage a caseload effectively, which includes regular updates for case management and utilization review, providing high-quality and timely care.
\n
• Coordinate and facilitate necessary support services for clients and their families, recording interactions and outcomes in clinical documentation.
\n
• Act as a liaison between clients and external referral sources, which may include legal, educational, and employment entities, to support clients' holistic recovery processes.

• Engage in crisis intervention, utilizing a multi-disciplinary team approach to address emergent client needs.

Patient Population / Client Focus\n

Your primary focus will be on patients experiencing co-occurring diagnoses, combining both mental health disorders and substance use issues. Qualifications\n

• A Bachelor’s Degree in Social Work or a related field is required; an LCSW license is mandatory for this position.

Compensation and Benefits\n
