Key Responsibilities\n

• Conduct comprehensive, virtual psychosocial assessments, determining clients' strengths and needs to inform individualized treatment approaches.

• Deliver evidence-based therapeutic interventions and counseling services tailored to individuals, families, and groups, utilizing modalities that resonate with client preferences and needs.

• Collaborate proactively with medical professionals, such as physicians and nurses at our affiliated hospice center, to ensure an integrated and holistic approach to client wellness.

• Maintain accurate documentation of client interactions, treatment progress, and therapeutic outcomes through our electronic health records system to support effective case management.

Qualifications\n

• A Master’s degree in Social Work from an accredited program.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in California.

• A minimum of two (2) years of post-licensure experience providing clinical social work services, particularly in settings related to mental health or hospice care.
