Key Responsibilities\n

• Conduct thorough mental health assessments and evaluations of patients in the hospice care setting, focusing on their psychological needs, history, and emotional state.

• Develop and implement individualized treatment plans that address mental health concerns, enhance coping mechanisms, and support familial dynamics during hospice care.

• Facilitate therapeutic sessions, including individual and family therapy, using evidence-based practices such as cognitive-behavioral therapy (CBT) and solution-focused therapy.

• Provide crisis intervention during acute emotional distress, utilizing trauma-informed practices and effective de-escalation techniques.

• Educate patients and their families about mental health conditions, treatment options, and community resources, promoting self-advocacy and informed decision-making.

• Maintain accurate clinical documentation, including treatment plans, progress notes, and compliance with all ethical and legal standards.

• Participate in regular team meetings and case reviews to discuss patient care, share insights, and enhance interdisciplinary collaboration.

• Stay abreast of relevant legislation and best practices in mental health, particularly as they relate to hospice care.

Patient Population / Client Focus\n

The primary patient population for this role includes individuals receiving hospice care, often facing serious terminal illnesses along with complex emotional and psychological challenges. Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current and valid Licensed Clinical Social Worker (LCSW) licensure in the state of California.

• A minimum of two years of post-licensure experience working in a mental health setting, preferably with a focus on hospice or palliative care.

Compensation and Benefits\n

The salary for this position ranges from USD 80,000 to 100,000 annually, commensurate with experience and qualifications.
