Key Responsibilities\n

• Conduct comprehensive assessments of seniors’ psychosocial needs, effectively evaluating emotional, social, and mental health factors.

• Develop personalized and responsive care plans in collaboration with clients and their families that address identified goals and preferences.
\n
• Provide counseling and emotional support, assisting clients through significant life transitions and challenges such as grief, illness, or social isolation.
\n
• Advocate for clients’ rights and access to essential community resources, facilitating their connection to supportive services and programs.
\n
• Work alongside an interdisciplinary team of nurses, caregivers, and other professionals to coordinate and monitor client care, ensuring all aspects of well-being are addressed.

• Conduct regular follow-up visits to assess the evolving needs of clients, ensuring their living conditions remain safe and conducive to their health.

• Support families by mediating communication and resolving conflicts that may arise in the caregiving process.

• Educate clients and families on available community resources, enriching their social engagement and quality of life.

• Maintain accurate and confidential documentation of client interactions, assessments, and care progress, adhering to ethical standards and guidelines.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Valid state licensure as a Licensed Clinical Social Worker (LCSW).

• Proficiency in electronic health record (EHR) systems and relevant computer applications.

• Exceptional interpersonal, communication, and counseling skills.
