Key Responsibilities\n

• Conduct thorough psychosocial assessments to identify patients' emotional, social, and financial challenges.

• Deliver individual and group counseling sessions focused on issues such as adjustment, grief, anxiety, and coping strategies.

• Develop and implement tailored discharge plans to ensure continuity of care following hospitalization.

• Coordinate with various community organizations to facilitate access to post-discharge resources and services.

• Provide crisis intervention and emotional support during critical moments for patients and their families.

• Maintain accurate and detailed documentation in compliance with all policies and regulatory requirements.

Patient Population / Client Focus\n

Qualifications\n

Compensation and Benefits\n

We offer a competitive annual salary ranging from USD 70,000 to 100,000, commensurate with experience and qualifications. In addition, you will receive a sign-on bonus as part of your compensation package.
