Work Environment\n

This position is designated as a remote role, providing you with the flexibility to manage your schedule while contributing to our detox center's mission. Regular work hours are set from 8:00 AM to 4:30 PM, Monday through Friday, with occasional evening or weekend work as needed based on client availability. While you will primarily work remotely, you should be prepared for on-call responsibilities and the need to adapt your schedule to ensure continuous care. Collaboration with our interdisciplinary teams will allow you to foster a dynamic connection with clients, empowering them throughout their recovery journey.

Key Responsibilities\n

• Conduct comprehensive assessments to identify the psychosocial needs of clients, applying your knowledge of emotional, mental, and social factors during evaluations.

• Collaborate with clients and their families to create individualized care plans that reflect their specific goals, challenges, and preferences.

• Provide counseling and emotional support to clients, assisting them in navigating life transitions and core challenges associated with recovery.

• Actively advocate for clients' rights, ensuring they have access to the essential resources and services available within the community.

• Work alongside a diverse team of healthcare professionals, including nurses and caregivers, to coordinate and enhance care effectively.

• Conduct regular follow-up visits to monitor client progress, assess living conditions, and address concerns related to safety or well-being.

• Facilitate effective communication and resolution of conflicts among family members involved in the clients' care, helping to promote a cohesive support system.

• Educate clients and their families about community resources, support groups, and programs that enhance social engagement and improve overall quality of life.

• Maintain meticulous documentation of client interactions, care plans, progress notes, and assessments to ensure accurate and timely record-keeping.

Your primary focus will be on clients facing substance use challenges, particularly seniors and individuals navigating mental health issues. Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
