This position allows you to perform your duties remotely while serving clients at a hospice center in Anaheim, California. Key Responsibilities\n

• Implement mandated reporting processes and execute the duty to warn as required by law, ensuring a safe environment for all clients.

• Conduct detailed assessments and evaluations to design tailored treatment plans that meet the varying needs of clients, addressing emotional, psychological, and social concerns.

• Collaborate effectively with physicians, nurses, and other therapists to develop comprehensive care plans that are holistic and respect the values of clients and their families.

• Monitor client progress by maintaining regular check-ins and adjusting treatment plans as needed to optimize therapeutic outcomes.

• Collect and analyze client outcome data to evaluate the effectiveness of interventions, using findings to inform continuous improvement practices.

• Maintain accurate, confidential client records and documentation using our electronic health record (EHR) system, ensuring compliance with all regulatory standards.

• Stay current with evidence-based practices in mental health therapy, participating in ongoing training and professional development opportunities.

Patient Population / Client Focus\n

Your primary focus will be on serving patients in a hospice environment, often facing terminal diagnoses. You will provide emotional support and mental health services to both patients and their families, helping them navigate complex feelings related to grief, loss, and acceptance. • A Master’s degree in Social Work, Counseling, or a closely related field.

• Current licensure in California as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
