Work Environment\n

This role is primarily remote, allowing for flexibility in managing your schedule while serving clients from the comfort of their own homes or hospice facilities. You will be part of an interdisciplinary team, working alongside healthcare professionals such as nurses, caregivers, and other social workers to ensure integrated care for our clients. Utilize telehealth tools as well as in-person visits when necessary to maintain a strong connection with those you serve.

Key Responsibilities\n

As a Licensed Master Social Worker at Clarity Pathways Counseling, your responsibilities will include:

• Conducting comprehensive assessments of clients to identify psychological, emotional, and social needs while developing tailored care plans that reflect their individual goals.

• Providing counseling and emotional support to seniors and their families to assist in navigating life transitions, including end-of-life considerations, with great sensitivity and respect.

• Advocating for clients, ensuring they have access to vital community resources and services that aid their well-being.

• Delivering educational resources to families about available social services, support groups, and programs that promote social engagement.
\n
• Regularly monitoring client well-being through home visits and virtual check-ins, documenting progress, and adjusting care plans as necessary.

• Collaborating effectively with interdisciplinary team members to enhance client care, participate in care planning meetings, and address any client or family concerns promptly.

• Resolving conflicts and mediating communication among family members involved in the care of senior clients, ensuring that everyone's voice is heard and attended to.

In this role, you will primarily serve an elderly population facing various life challenges, including chronic illnesses and end-of-life issues. You will engage with clients who have unique psychological and emotional needs, and your work will directly contribute to improving their mental health and overall quality of life during their final stages.

Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Master Social Worker (LMSW).

• A minimum of two years of experience in a social work role, particularly within a hospice, community center, or geriatric setting.

• In-depth knowledge of social work principles and best practices, particularly related to aging and end-of-life issues.

• Strong assessment, counseling, and communication skills, with a demonstrated ability to empathize with clients and their families.

Preferred Experience\n

Compensation and Benefits\n
