You will work closely alongside health care providers in a Hospice Center, collaborating on patient assessments and treatment plans while utilizing electronic health record systems to document patient progress and care interventions. Key Responsibilities\n

• Conduct comprehensive biopsychosocial assessments tailored to the unique mental health, substance abuse, and medical needs of patients in the Hospice Center.

• Provide a wide range of behavioral health services including individual and group counseling, intake assessments, crisis intervention, treatment planning, and collaboration with healthcare providers for referrals to community resources.

• Diagnose mental health conditions and develop personalized treatment plans that respect the values and aspirations of each patient.

• Engage in ongoing documentation of patient encounters, including progress notes and suicide risk assessments, ensuring accuracy and confidentiality in line with legal and ethical standards.

• Serve as a liaison between patients and community resources, assisting in connecting them with local agencies and medication assistance programs to enhance their support networks.

• Collaborate closely with the Quality Improvement team to gather and analyze data, implementing processes that aim to enhance service delivery and patient outcomes.

Qualifications\n

• Must possess a valid state license as a Licensed Clinical Social Worker (LCSW).

• A Master’s degree in Social Work is required, with a minimum of three (3) years of post-licensure experience in a clinical setting.

• Demonstrated knowledge of therapeutic techniques, crisis intervention strategies, and the ability to make independent clinical decisions is crucial.

Compensation and Benefits\n
