Family Support Advocate | Home Visits & Resources
Community Of Hope Inc.
Community of Hope in Washington, DC is seeking a Family Support Specialist to provide direct support and education to families through home visits, resource coordination, and advocacy. This role is hybrid and based at the Conway Health and Resource Center in SW DC. You will maintain a caseload of 15-25 families, develop service plans, document observations, and coordinate medical care while connecting families with local resources. #J-18808-Ljbffr Community Of Hope Inc.
