Sign-on Bonus: In-Home MA Traveler, Nurse Practitioner or PA

Job Summary Full‑time Clinician (Nurse Practitioner or Physician Assistant) providing in‑home health evaluations across Massachusetts and potentially other states. The role requires extensive travel, up to 100% of the time, including overnight stays, car rentals, or flights. Key Responsibilities Conduct 45‑minute in‑home visits, including medication and medical history review, physical evaluation, and optional diagnostic tests. Maintain patient rapport and provide education and preventive guidance. Travel locally and within licensed states to cover assigned counties, up to 100% of the time. Perform virtual visits when indicated. Use the Signify Health clinical workflow and iPad for documentation. Provide diagnostic and preventive services such as spirometry and diabetic eye exams as ordered. Receive per‑completion incentive pay and additional per‑visit incentive. Qualifications Active, unrestricted license(s) in the coverage area(s). Board certification (required). Ability to obtain and maintain multi‑state licenses as required.
