Social Worker - Mandarin/Cantonese Speaking | Join Our Compassionate Team!

At SC Evergreen Health Management Inc., located in Rosemead, CA, we are dedicated to providing exceptional care and support through our Community-Based Adult Services (CBAS) program. We’re looking for a compassionate and skilled Social Worker fluent in Mandarin (Cantonese preferred) to join our team full-time and help us create meaningful, lasting impacts in our community.

About the Role

As a Social Worker, you’ll play an integral role in supporting seniors and their caregivers. From welcoming new participants to developing individualized care plans in collaboration with our interdisciplinary team, you’ll be at the heart of our mission to provide person-centered care. Key Responsibilities

• Deliver excellent customer service as a full-time staff member during all operational hours.
• Welcome and orient visitors, families, and new participants to our staff, program, and facilities.
• Gather medical and financial information to establish participant eligibility and complete the enrollment process.
• Develop individualized care plans alongside the interdisciplinary team, incorporating person-centered planning and participant/family input.
• Conduct home environment assessments prior to enrollment to evaluate living arrangements and accessibility.
• Provide counseling, education, and crisis intervention to participants and families, ensuring continuity of care.
• Act as a liaison between participants/families and social service agencies, offering referrals and information as needed.
• Facilitate support groups for participants, families, and caregivers.
• Organize outreach programs for participant recruitment and oversee the intake and enrollment process.
• Maintain accurate and timely documentation, including progress notes and treatment plans.
• Assist in staff training and development, as well as internal utilization review committees.
• Perform other duties as assigned by the Program Director.

Required Skills and Qualifications

Education and Experience
- Master’s Degree in Social Work, Psychology, Gerontology, or Counseling from an accredited school with 1 year of social services experience in aging, health, or long-term care; OR
- Bachelor’s Degree in Social Work from an accredited school with 2 years of social services experience.

Other Qualifications
- Bilingual: Mandarin required , Cantonese preferred.
- Experience in case management with elderly populations.
- Knowledge of the physical, mental, and social needs of older adults.
- Ability to work effectively in a multi-disciplinary team environment.
- Strong written communication skills and a collaborative, team-oriented mindset.
