Home Health Physical Therapist – North Kansas City, Missouri (Travel) Supplemental Health Care is hiring Home Health Physical Therapists (PTs) to provide compassionate, patient‑centered care in North Kansas City, Missouri and the surrounding areas. The role is a 13‑week travel assignment with 40 hours per week. Specialty Physical Therapist Discipline Therapy Start Date ASAP Duration 13 weeks Work Hours 40 hours per week (8‑hour shifts, 5–6 visits per day) Employment Type Travel Qualifications Active, unrestricted Physical Therapist (PT) license in the state of assignment. Current BLS certification (AHA). At least 1 year of recent home health experience within the last 2 years. Experience conducting PT evaluations and developing therapy plans. Familiar with Medicare‑certified home health guidelines and OASIS‑E documentation. Experience with EMRs such as Homecare Homebase, NetSmart, or Epic. Clear background check, driving record, and up‑to‑date health clearance (TB, immunizations). Reliable transportation with valid auto insurance and ability to travel between patient homes. Contract Details Weekly pay: $2,120 – $2,305 per week. Paid by the hour, not by the visit. 5–6 visits per day, 45–90 minutes each based on complexity. Travel time factored into productivity expectations. Key Responsibilities Perform initial and ongoing PT assessments and develop individualized plans of care. Provide skilled interventions for strengthening, endurance, pain management, and fall prevention. Conduct gait and balance evaluations, home safety assessments, and mobility training. Educate patients and caregivers on exercise programs, adaptive techniques, and safety strategies. Collaborate with RNs, OTs, SLPs, and physicians for coordinated, interdisciplinary care.
