Schedule : Regular office hours with periodic on-call availability, including initial client assessments, RN Case Management, Care Plan Reviews, Supervisory Visits and Community Outreach/Lectures/Demos. 

Position Summary

North Star Home Care LLC is seeking a highly organized, compassionate, and results-driven RN Community Manager to support the daily operations and continued growth of our non-medical home care agency.

This is a multifaceted position combining office administration, prospective client intake, on-call initial assessments, sales, relationship-building, and senior community outreach.

The role serves as an important point of contact for prospective clients and their families, helping them understand North Star Home Care's services and guiding them through the initial intake and assessment process. The position will also represent the agency within the senior care community and promote technologies and resources offered through our partner product.

The ideal candidate is comfortable moving between administrative work, client and family communication, community networking, sales conversations, and in-home assessments.

Key Responsibilities

Office Management & Administrative Operations

• Assist with the administrative operations
• Answer and manage incoming client and community inquiries in a professional and timely manner.
• Maintain accurate administrative, client, referral, and outreach records.
• Assist with client intake documentation during initial assessments and ensure required records are properly completed and maintained.
• Help management maintain efficient workflows, documentation systems, various operational procedures, onboarding, compliance tasks, and skills checks for new employees.
• Handle confidential client and business information with discretion and follow applicable privacy and agency requirements.
• Handle RN Case Management

Prospective Client Intake & Initial Assessments

• Respond to inquiries from prospective clients, families, referral partners, and community organizations.
• Conduct initial telephone consultations to understand the prospective client's needs and determine appropriate next steps.
• Schedule and conduct initial client assessments, including in-home assessments when assigned.
• Participate in on-call shifts or on-call assessment schedule, including responding to prospective clients when timely assessments are needed.
• Gather relevant information regarding the prospective client's needs, living situation, requested services, schedule, preferences, and other information required by agency procedures.
• Clearly explain North Star Home Care's non-medical home care services and our partner's virtual caregiver product to prospective clients and family members.
• Coordinate assessment findings and potential service arrangements with agency leadership using preferred template
• Follow up with prospective clients and families following assessments and inquiries.
• Assist with transitioning qualified prospective clients from inquiry to service initiation.

Sales & Business Development

• Conduct outbound calls to prospective clients, families, referral sources, and community organizations.
• Follow up on inquiries and leads generated through marketing, referrals, community events, and agency outreach.
• Represent North Star Home Care in marketing events and community gatherings like senior expos, seminars, and presentations.
• Schedule presentations and distribute approved educational materials on North Star Home Care services and our partner product/service
• Educate prospective clients about North Star Home Care's services and how non-medical home care may support independence and aging in place.
• Educate established and prospective clients on North Star Home Care’s Premier Services as well as our partner product's advantages.
• Develop professional relationships with organizations and communities serving older adults.
• Identify opportunities for new client referrals and community partnerships.
• Maintain organized records of leads, referral sources, follow-up activities, and outcomes.
• Support agency marketing initiatives and community-based business development activities.
• Represent North Star Home Care professionally when communicating with community partners and prospective clients.

Senior Community Outreach

The RN Community Manager will serve as a community-facing representative of North Star Home Care LLC and may conduct outreach to organizations and professionals such as:

• Senior living communities
• Independent living communities
• Assisted living communities
• Senior centers
• Hospitals and discharge-planning contacts
• Rehabilitation facilities
• Social workers and case managers
• Physician and provider offices
• Community organizations
• Faith-based organizations
• Senior programs and community events
• Other referral partners serving older adults and their families

On-Call Responsibilities

Because home care needs do not always arise during normal business hours, this position may include scheduled on-call responsibilities.

Ideal candidate may be expected to:

• Respond to prospective client inquiries while on call.
• Conduct or coordinate initial assessments or on-call shifts when appropriate.
• Communicate with management regarding new client opportunities.
• Document on-call communications and follow up during normal business hours.
• Travel to prospective clients' residences when an in-person assessment is necessary and assigned.
• Follow established escalation procedures for situations outside the scope of non-medical home care.

Qualifications

Required

• Associate’s Degree in Nursing – Active RN Licensure
• Previous experience in office administration, customer service, sales, healthcare, home care, senior services, or a related environment.
• Previous experience conducting client intake or non-clinical assessments.
• Excellent verbal and written communication skills.
• Strong interpersonal and relationship-building abilities are important.
• Comfortable speaking with older adults, family members, healthcare professionals, and community organizations.
• Comfortable making outbound sales and outreach calls.
• Strong organizational and time-management skills are important.
• Ability to work independently and manage multiple priorities.
• Professional, dependable, compassionate, and customer-service-oriented.
• Comfortable learning and teaching the community our technology-based senior care solutions.
• Ability to maintain confidentiality and appropriately handle sensitive information.
• Ability to travel locally for assessments, outreach activities, meetings, presentations, and community events.
• Valid and current BLS certification
• Able to lift 40lbs
• Reliable transportation and a valid driver's license are essential functions of the role.

Preferred

• Experience in home care, senior care, healthcare administration, assisted living, or community-based services.
• Previous experience conducting client intake or non-clinical assessments.
• Sales, marketing, community outreach, or business development experience.
• Experience building relationships with senior communities and healthcare referral sources.
• Familiarity with CRM, home care management, scheduling, or electronic documentation systems.
• Experience presenting products or services to individuals and community organizations.

Physical & Travel Requirements

The position may require:

• Regular computer and telephone use.

• Reasonable mobility necessary to perform the essential responsibilities of the position, with reasonable accommodations as applicable
