Hospice Registered Nurse Case Manager

Lifecare Home Health Family

LifeCare Home Health Family is seeking a registered nurse case manager to perform admissions and provide in-home nursing care for patients served across multiple states. You will coordinate with the interdisciplinary team, patients and families, and referring agencies to implement and monitor the care plan. The role requires an active RN license, at least two years of clinical RN experience and one year in hospice, along with valid driver's license and strong communication skills. #J-18808-Ljbffr
