Responsibilities:

• Serves in a managerial/supervisory capacity to conduct performance management activities and ensure effective onboarding, offboarding, and staff training.
• Establishes and reviews work assignments and priorities.
• Implements performance improvement strategies and resolves employee-related issues in collaboration with program management and Human Resources.
• Ensures staff complete required training and adhere to workplace safety standards.
• Conducts performance monitoring and annual appraisals.
• Establishes clear performance expectations and addresses performance deficiencies in a timely manner in accordance with state guidelines.
• Documents underperformance appropriately.
• Supports employee growth through regular feedback, coaching, and professional development opportunities.

B. Direct Patient Care


Provides comprehensive patient care within the licensed scope of practice and in accordance with local and state policies and procedures. Services may include, but are not limited to:

• Child Health
• Adolescent Health
• Adult Health
• Family Planning
• Immunizations
• Travel Clinic Services
• Sexually Transmitted Disease (STD/STI) Services

Job Duties

• Provides comprehensive patient care consistent with professional standards and established clinical protocols.
• Obtains complete medical histories and documents findings in a systematic, accurate, and concise manner.
• Reviews patient history and evaluates patient status during each visit.
• Interviews patients to verify medical history, assess current complaints, treatment history, systems review, and family/social history.
• Performs comprehensive and accurate physical examinations.
• Orders, performs, and interprets routine laboratory and diagnostic tests.
• Reviews laboratory and diagnostic results and modifies plans of care as clinically appropriate.
• Administers and prescribes medications, including controlled substances, within the guidelines of the collaborative practice agreement and applicable regulations.
• Develops and implements individualized health promotion, disease prevention, and health maintenance plans.
• Provides health education and counseling to patients and families.
• Performs periodic health assessments and preventive health screenings.
• Provides ongoing clinical management of patients.
• Co-manages patients with selected health complications in collaboration with the supervising physician.
• Develops, implements, and monitors individualized treatment and care management plans.
• Arranges referrals to specialists, members of the healthcare team, community providers, and other community resources as appropriate.
• Coordinates continuity of care and follows up on referrals, diagnostic testing, and treatment plans.
• Performs nursing assessments and may direct other clinical staff in carrying out patient care activities within the scope of practice.
• Performs laboratory testing and clinical measurements as required.
• Maintains complete, accurate, and timely patient documentation in accordance with regulatory, legal, and organizational standards.
• Participates in quality improvement initiatives, infection prevention activities, and patient safety programs.
• Maintains professional competency through continuing education and participation in professional development activities.
• Performs other related duties as assigned within the scope of licensure and organizational needs.
