Building Name: UVMMC - Medical Center - Main Campus Location Address: 111 Colchester Avenue, Burlington Vermont Regular Department: Interventional Radiology Full Time Standard Hours: 40 Biweekly Scheduled Hours: 80 Shift: Day-8Hr Primary Shift: 7:30 AM - 4:00 AM Weekend Needs: None Salary Range: Min $45.45 Mid $59.97 Max $74.48 Recruiter: Brianna Foley JOB DESCRIPTION: The Care Coordinator is responsible for coordination of activities related to a specific patient population and the administration of direct patient care for a specific unit and shift. The Coordinator functions as a leader of multidisciplinary teams, providing and coordinating care to patients and families. Concurrent utilization and service review, consultation, patient and family education, promotion of continuity of care, preceptorship and committee work are required at this level of practice. EDUCATION: Current RN licensure or compact licensure recognized by the State of Vermont is required. ADN required, BSN preferred. EXPERIENCE: Three years of relevant RN experience This is a bargaining union position.
