This Registered Nurse (RN) position is in Mental Health services at the Providence VA Medical Center, located at the Providence, RI location. This position is a full-time position at 40 hours per week. The Mental Health Outpatient RN delivers safe, evidence-based, patient-centered care to Veterans receiving psychiatric and behavioral health services in an outpatient setting. This role supports comprehensive assessment, treatment planning, medication management, crisis intervention, and care coordination to foster recovery, independence, and improved mental health outcomes.

Duties include, but not limited to:

• Participates in warm hand-off, care coordination, liaising with Inpatient MH Units to facilitate continuity of care, and responds to psychiatric and medical emergencies as directed.
• Documents nursing triage assessments, risk screens, clinical reminders, and patient plan of care in accordance with facility policy.
• Maintains professional boundaries to protect patient vulnerabilities and act in the best interest of the patient.
• Provides patient centered and recovery-oriented care and maintains self-awareness while caring for patients living with MH conditions. Reports and assists and/or directs safety concern resolution and collaborates with program leadership to help improve patient and program outcomes; and directs team response to psychiatric and medical emergencies.
• Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care.
• Provides peers with formal or informal constructive feedback for improvement.
• Supports colleagues and other nurses through knowledge sharing to provide safe, quality nursing care.
• Serves as the primary contact for Veterans.
• Provides direct patient care, including medication administration, injections, health teaching, and symptom management for Veterans engaged in Behavioral Health Interdisciplinary Program (BHIP) services.
• Participates as an integral member of the interdisciplinary BHIP team collaborating with psychiatrists, psychologists, social workers, and other providers in treatment planning and ongoing evaluation.
• Supporting care coordination and case management functions, including follow-up on treatment plans, coordination with specialty services, and connecting Veterans to internal and community resources.
• Performs crisis intervention and de-escalation, responding to acute changes in mental status, and activating emergency protocols when indicated to maintain safety for Veterans and staff.
• Monitoring and documenting patient progress, responses to treatment, side effects, and changes in mental health status, ensuring timely communication with the BHIP team.
• Facilitating communication among providers, ensuring continuity across levels of care such as primary care, inpatient psychiatry, residential programs, and community partners.
• Participating in performance improvement initiatives, population health strategies, safety huddles, and evidence-based program implementation within the Mental Health Service.
• Monitors Performance Measures, wait times, incomplete encounters, and outcome measures for BHIP team
• Maintaining compliance with VHA directives, Joint Commission standards, infection control practices, and all nursing practice policies.

Pay: Competitive salary, regular salary increases, potential for performance awards
