In this resident-centered role, you will assess functional abilities, create and implement individualized treatment plans, and deliver evidence-based interventions that promote safety, independence, and quality of life for seniors in independent living, assisted living, and memory care. You will document care in EMRs, collaborate closely with nursing and care teams, educate families and caregivers, and support a culture of compassion, community, and personal growth within our supportive senior living environment.
