Description
  • Conduct thorough assessments of patients in both inpatient and outpatient settings, crafting personalized treatment plans tailored to individual needs
  • Implement a range of physical therapy interventions, emphasizing mobility, strength, balance, and overall functional improvement
  • Work collaboratively with nurses, physicians, case management, and other rehabilitation professionals to maximize patient outcomes
  • Guide patients and their families through therapeutic goals, discharge plans, and at-home exercise routines
  • Maintain meticulous documentation of evaluations, treatments, and patient progress in compliance with facility and regulatory standards
  • Engage in care team rounds and interdisciplinary discussions to ensure seamless, coordinated care
Education
  • Doctorate in Physical Therapy (DPT) or equivalent degree from an accredited institution
Certifications
  • Current state licensure as a Physical Therapist
  • CPR certification preferred
  • Any specialized certifications (i.e., neurological, geriatric) are advantageous but not mandatory
Skills
  • Relevant previous experience is strongly preferred
  • Ability to manage patients with complex medical and mobility challenges
  • Exceptional communication and teamwork abilities
