- Conduct thorough assessments of patients in both inpatient and outpatient settings, crafting personalized treatment plans tailored to individual needs
- Implement a range of physical therapy interventions, emphasizing mobility, strength, balance, and overall functional improvement
- Work collaboratively with nurses, physicians, case management, and other rehabilitation professionals to maximize patient outcomes
- Guide patients and their families through therapeutic goals, discharge plans, and at-home exercise routines
- Maintain meticulous documentation of evaluations, treatments, and patient progress in compliance with facility and regulatory standards
- Engage in care team rounds and interdisciplinary discussions to ensure seamless, coordinated care
- Doctorate in Physical Therapy (DPT) or equivalent degree from an accredited institution
- Current state licensure as a Physical Therapist
- CPR certification preferred
- Any specialized certifications (i.e., neurological, geriatric) are advantageous but not mandatory
- Relevant previous experience is strongly preferred
- Ability to manage patients with complex medical and mobility challenges
- Exceptional communication and teamwork abilities
- Comfortable with digital charting and clinical documentation systems
