Essential Duties
- Provide case management services to youth and families involved with the Youth Justice system to promote public safety, accountability, rehabilitation, and positive youth development, in accordance with Wisconsin Statutes, court orders, and Department policy.
- Conduct comprehensive assessments of youth and families through interviews, school visits, home visits, collateral contacts, and review of records to evaluate strengths, needs, risks, and protective factors.
- Review and process youth justice referrals received from law enforcement; conduct intake inquiries with youth and families; apply approved assessment tools to inform recommendations; and participate in case staffing and decision-making regarding appropriate disposition or court action.
- Develop, implement, and monitor individualized case plans in collaboration with youth, families, and service providers, including identifying goals, required actions, timelines, and appropriate interventions.
- Provide ongoing case management and supervision services for youth involved through informal agreements or court orders, including monitoring compliance with conditions, responding to violations, and initiating appropriate court action when necessary.
- Respond to crisis situations involving youth safety, community safety, or court violations; coordinate with law enforcement, detention facilities, schools, and other partners; and provide consultation and authorization regarding custody or detention decisions as required.
- Prepare court-related documentation, reports, and recommendations; attend hearings; and provide written and oral testimony as required throughout the life of a case.
- Maintain accurate, timely, and complete case documentation in the State of Wisconsin's automated child welfare and youth justice systems, including case notes, assessments, service plans, reviews, referrals, and closure documentation.
- Coordinate and participate in multidisciplinary team meetings, service staffings, and consultations with schools, treatment providers, alternate care providers, community agencies, and internal County partners.
- Provide direct support to youth and families, including skill building in areas such as emotional regulation, decision-making, communication, and problem-solving, and connect families to appropriate community services and resources.
- Monitor and review youth progress, service engagement, restitution and community service obligations, and compliance with court or informal agreements; adjust case plans as needed based on assessment and outcomes.
- Conduct required reassessments, reviews, and evaluations at designated intervals and at case closure, using approved assessment tools and documentation standards.
- Maintain regular consultation with supervisory staff regarding case direction, legal requirements, and complex decision-making.
- Participate in unit, division, department, and County meetings; interdisciplinary workgroups; and required training, professional development, and licensure maintenance activities.
- Apply trauma-informed principles, cultural humility, and ethical standards in all interactions with youth, families, colleagues, and community partners.
- Perform administrative and program support duties necessary to carry out assigned responsibilities, including completion of required reports, statistics, and documentation.
- Perform other related duties as assigned to support the mission, programs, and operations of the Health and Human Services Department.
- Knowledge of applicable federal and state laws, Wisconsin Statutes, administrative rules, court procedures, and Department policies governing youth justice, juvenile delinquency, and court-involved social work practice.
- Knowledge of youth development, adolescent behavior, family systems, trauma, cultural diversity, and the social and environmental factors that influence youth behavior and decision-making.
- Knowledge of assessment, intervention, and case management principles applicable to youth justice and court-involved populations, including use of approved assessment tools and evidence-informed practices.
- Skill in conducting comprehensive assessments of youth and families through interviews, school and home visits, collateral contacts, and review of records to evaluate strengths, risks, needs, and protective factors.
- Skill in developing, implementing, monitoring, and updating individualized case plans in collaboration with youth, families, and service providers.
- Skill in providing case management and supervision services for youth under informal agreements or court orders, including monitoring compliance, responding to violations, and recommending appropriate interventions or court action.
- Skill in preparing accurate, timely, and well-organized court reports, recommendations, and documentation, and in providing written and oral testimony when required.
- Skill in crisis response and de-escalation, including sound decision-making when addressing youth safety, community safety, or court-related concerns.
- Skill in coordinating services and activities that cross unit, departmental, and agency lines, including collaboration with courts, law enforcement, schools, detention facilities, treatment providers, and community organizations.
- Effective written and verbal communication skills, including the ability to explain legal processes, program expectations, and case requirements clearly to youth, families, and partners.
- Skill in maintaining accurate, timely, and confidential documentation in the State of Wisconsin's automated child welfare and youth justice systems in accordance with legal, professional, and Department standards.
- Demonstrated commitment to confidentiality and ethical handling of sensitive information in accordance with legal requirements, professional standards, County policies, and the NASW Code of Ethics.
- Sound professional judgment, emotional composure, and professionalism when working in stressful, complex, or confrontational situations.
- Capacity to plan, organize, prioritize, and manage a caseload effectively while meeting deadlines, documentation standards, and statutory requirements.
- Ability to work independently and collaboratively within interdisciplinary teams, follow supervisory direction, and participate in consultation and quality improvement activities.
- Commitment to trauma-informed practice, cultural humility, equity, and respectful engagement with youth, families, and community partners.
- Possession of a valid driver's license, automobile insurance, reliable access to a vehicle, and the ability to travel within and outside Sheboygan County as required.
- Work is performed in a combination of office settings within Health and Human Services facilities and community-based environments, including schools, client homes, court facilities, detention settings, and other off-site locations.
- Position requires regular travel throughout Sheboygan County to conduct home visits, school visits, court appearances, meetings, and community-based services; travel may involve exposure to typical driving hazards.
- Community-based work may involve exposure to varying environmental conditions, including inclement weather, temperature extremes (heat and cold), snow and ice, uneven surfaces, noise, wetness or humidity, and limited lighting conditions.
- Position involves frequent interaction with youth, families, school personnel, law enforcement, court officials, service providers, coworkers, and members of the public, requiring professionalism, effective communication, and appropriate boundaries.
- Employees may occasionally interact with individuals experiencing crisis, trauma, heightened emotions, or confrontational behavior and must maintain professionalism, situational awareness, and personal safety.
- Work involves a range of physical activities, including sitting, standing, walking, climbing stairs, stooping, kneeling, reaching, grasping, writing, and performing repetitive motions.
- Position may require exerting up to 30 pounds of force occasionally and/or a negligible amount of force continuously to move objects such as files, materials, or supplies.
- Hearing sufficient to communicate effectively with individuals in person, by telephone, and in group settings is required.
- Vision sufficient to read printed and electronic materials, view computer screens, review documentation, and safely operate a motor vehicle is required.
- Position requires the use of standard office equipment and technology, including computers, phones, and related software systems.
- Community settings may present exposure risks including communicable diseases, household pets or animals, environmental hazards, and situations involving volatile or unpredictable behavior.
- Risk of exposure to blood-borne pathogens is considered low to moderate.
