Job Title: Registered Nurse - Case Management Profession: RN (Registered Nurse) Specialty: Case Management Duration: 13 weeks Shift: 8-hour days, Monday through Friday, with weekend rotation Hours per Shift: 8 hours Experience: Minimum of 2 years in inpatient case management License: Indiana RN License or compact Certifications: BLS Certification preferred, Certification in Case Management preferred Must-Have: - Competency with MCG and InterQual - Ability to work independently - Ability to identify work that needs to be done and follow through to completion without prompting - Ability to work collaboratively with the care team - Capability to complete a comprehensive patient assessment driving discharge - Proficiency documenting in a comprehensive, accurate, and timely manner Description: The position requires a competent Registered Nurse with a minimum of 2 years of experience in inpatient case management. The role involves collaborative work with the care team and completion of comprehensive patient assessments to drive discharge. The individual must be proficient in completing discharge planning that expedites patients' transitions to the next level of care. Comprehensive and accurate documentation is essential. Team members will be assigned primarily to one unit but may need to cover other units as needed. Experience in hospital utilization management is critical. The position involves floating to other units but focuses exclusively on inpatient case management work. The team consists of one or two RN case managers assigned to various units, each unit ranging from 20 to 36 beds. The dress code is business casual or eggplant colored scrubs top and bottom, with closed-toe shoes. General job duties include utilization review, stay progression, and comprehensive patient assessment documentation.
