Collect, assess, and document patient healthcare data. Lead and inspire the interdisciplinary team to design a realistic and measurable plan of care. Serve as a role model to prioritize care based on collected data. Document assessment and reassessment in medical records. Design interventions for patient care coordination in a timely manner. Educate patients and families regarding health status and care plans. Collaborate with the interdisciplinary team to modify expected outcomes. Evaluate the patient's progress towards expected outcomes. Comply with professional standards and regulatory requirements.
