About This Role The Per Diem Clinical Oncology Nurse Navigator is a registered nurse with oncology-specific clinical knowledge, providing individualized assistance to patients, families, and caregivers throughout the cancer continuum. This role is ideal for those who excel in patient advocacy and care coordination. Highlights Pay: $70/hour Location: Long Island City, NY Facility: Mount Sinai Queens Department/Unit: Cancer Center - MSQ Schedule: Per Diem, Day shift What You'll Do Deliver patient care using the professional practice model of Relationship-Centered Care. Organize and lead day-to-day patient care coordination. Assist patients in accessing cancer care and navigating healthcare systems. Assess barriers to care and engage patients in creating solutions. Identify appropriate resources responsive to patient needs. Collaborate with the interdisciplinary healthcare team to develop and evaluate care plans. Establish and implement goals, objectives, and policies for nursing care delivery. Educate patients and caregivers on cancer treatment and health system navigation. Follow up with patients to support adherence to treatment plans.
