THP Plus Case Manager: Youth Housing & Life Skills

Walden Family

Walden Family Services in Riverside, CA seeks a dedicated case manager to support former foster youth. You will coordinate with placing agencies, families, and adjunct treatment to help youth achieve their goals. This role does not include after-hours or on-call duties. Responsibilities include pre-placement appraisals, intake and needs/services plans, weekly client consultations, progress documentation, and collaboration with supervisors. #J-18808-Ljbffr Walden Family
