Collaborate with the interdisciplinary team, residents, families, and community partners to ensure quality care and smooth care transitions. Assist residents and families with community services, financial assistance, discharge planning, and other support resources. Identify safety concerns, respond to changing resident needs, and advocate for resident rights while maintaining compliance with healthcare regulations. What Minimum Requirements You’ll Need Licensure / Certification / Registration: Master Social Worker, preferred. Education High school diploma/GED with 2 years of experience, or Associate's degree, or Bachelor's degree required. Work Experience 3 years of experience preferred. What Additional Preferences We're Seeking Long term care experience highly preferred. Clinical Social Worker Licensed highly preferred.
