Description

• Conduct assessments in either an inpatient or outpatient environment and create tailored treatment plans for each patient
• Implement physical therapy interventions targeting mobility, strength, balance, and overall function
• Collaborate with a team of nurses, physicians, case managers, and other rehabilitation professionals to enhance patient outcomes
• Educate patients and their families regarding therapeutic objectives, discharge strategies, and at-home exercises
• Document evaluations, treatment methodologies, and patient progress according to facility protocols and regulations
• Engage in care team meetings and interdisciplinary discussions to ensure a seamless continuum of care

Education

• Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification preferred
• Specialized certifications (e.g., neurological, geriatric) are advantageous but not mandatory

Skills

• Previous experience in a clinical setting preferred
