Job Description

Schedule:

• 13-week contract, with possibility for extension
• Full-time, Monday through Friday
• Potential on-call requirements
• Structured daily visit goals

Key Responsibilities:

• Deliver skilled, compassionate in-home nursing care
• Complete thorough patient assessments and document findings via OASIS accurately and promptly
• Provide wound care, medication administration, education, and disease monitoring
• Create individualized care plans in collaboration with physicians and interdisciplinary teams
• Ensure prompt, effective communication of any changes in patient condition
• Maintain accurate, timely records using HomeCare HomeBase (preferred)

Qualifications:

• Current, active RN license
• At least one year of recent RN home health experience
• Proficiency with OASIS is required
• Familiarity with HomeCare HomeBase is strongly preferred
• Reliable transportation and a valid drivers license
• Confidence in independently managing a full patient caseload with daily visits
