PRN Physical Therapist - In-House SNF Avir at The Lakes is excited to offer an opportunity for a PRN Physical Therapist (PT) to join our in-house rehabilitation team . This role is ideal for a clinician who is passionate aboutdelivering high-quality, resident-centered care in a collaborative skilled nursing environment. Must have weekend availability. Position Overview As a Physical Therapist , you will evaluate residents, develop personalized treatment plans, and provide skilled therapy services aimed at improving mobility, strength, balance, and functional independence. Key Responsibilities Evaluate residents' physical conditions and functional limitations Develop and implement individualized treatment plans using evidence-based practices Provide therapeutic interventions to restore mobility and reduce pain Monitor resident progress and adjust treatment plans as needed Educate residents, families, and caregivers on exercises and mobility strategies Complete accurate and timely documentation per facility and regulatory standards Collaborate with the interdisciplinary team to ensure optimal resident outcomes Maintain compliance with all safety and regulatory requirements Qualifications Active Texas Physical Therapist (PT) license In good standing with all licensing boards and regulatory agencies SNF or long-term care experience preferred Familiarity with Net Health documentation system a plus Strong communication and clinical skills Recruiter Contact information Call-Text-Email Jami at View phone number on click.appcast.io / View email address on click.appcast.io
