Backed by over 40 years of healthcare staffing expertise, SHC is here to help you find the right fit for your career.Qualifications:Must be a graduate of an approved school of professional nursing and hold an active RN licenseCurrent BLS (AHA) certification2 years of nursing experience and at least 1 year of experience in Home HealthOasis experienceHomeCare HomeBaseAdditional Requirements:Must possess a valid state driver's license and automobile liability insuranceMust be able to drive an automobile in all types of weather conditionsMust have an RN license in the State of employmentSupervisory experience is preferredClinical Manager Contract Details:Pay: $2,260 - $2,487 per week*Duration: 13-week contract, with potential to extend8 hours in office position40 hours per weekKey Responsibilities:Ensure that the client's plan of care is individualized and appropriate, executed as written and reassessed by the appropriate health care processional when there is a significant health status change in the client's condition, at the physician's request, and after hospital dischargeComplete review of evaluation packets, OASIS and 485sResponsible for Referral Intake and Management to ensure that clients receive assessment visits and that these visits are scheduled and performed timelyWe understand home health care is unique and requires the right support for success.
