Key Responsibilities

• Conduct thorough health assessments for individuals and families, evaluating both physical and mental health needs.
• Diagnose, manage, and treat a variety of acute and chronic medical conditions, emphasizing preventive care and health education.
• Develop and implement culturally sensitive healthcare plans that address the specific needs of your patient population.
• Collaborate with local community organizations to establish health education programs that empower individuals with knowledge about their health.
• Advocate for patients' rights, ensuring they have access to required healthcare services, and assist with navigating the healthcare system.
• Coordinate and manage referrals to specialists and ensure comprehensive follow-up care for patients, facilitating seamless transitions in their healthcare.

Qualifications

• Current certification as a Nurse Practitioner.
• An active nursing license in the state of Texas.

Compensation and Benefits

• We offer a competitive salary range of $120,000 - $130,000 annually, commensurate with experience and qualifications.
