Home Service Worker

Position SummaryResponsible for providing home services with full adherence to the limitations established by law for a home service workers. Home Service duties will vary per client. Client care plan will be created and monitored by the Nurse Manager for the case.Pay range: $17.50 - $18.50 per hour depending on CNA certificationBenefits may include depending on full or part-time status:Access to health, dental and vision benefits401k retirement plan with company matchEducation Assistance ProgramAccess to other benefits like Short Term Disability and Accident coveragePaid Time OffEssential Duties and ResponsibilitiesMaintain current contact information with the scheduling office at all timesMaintain a personal cell phone in working order that has smart-phone technology to accept texts and access internet based website services and communications.Adhere to scheduling and communicationThe following list of duties which fall within the skilled home service worker scope of services.Assist with and may perform activities of daily living.Personal care including bathing, non-prescription lotion application, dressing, toileting, hair care, non- prescription elastic stocking application and shaving with electric razor only.Assist with household chores, including cooking, meal preparation, light cleaning and laundry.Assist in completing activities such as shopping and appointments outside of the home.Assist with meals and feeding of the client as long as the client can swallow independently and be positioned upright.Oral hygiene care.Medication reminders.Encouragement to do home exercise program established by a physical therapist.Assistance with ambulation, including safe transfer techniques and ambulationAssistance with transfers if client is able to bear weight on lower extremities.ToiletingCompanionship.Recognize and report emergencies and demonstrate emergency procedures appropriately.Adhere to basic infection control procedures in the nursing environment.Observation, documentation, and reporting on mental and functional ability changes.General ResponsibilitiesAdhere to the policies and procedures of the business with the highest practical standardsParticipate in and maintain current on all training and education required by the companyCommunicate effectively with all members of the staff and business partnersProvide feedback to others for continuous quality improvement of the entire organizationOpenly accept feedback from others for the continuous quality improvement of the entire organizationEscalate issues when there is appearance that business standards of quality are not presentComply with all rules and regulations of the organization, including HIPAA and safetyPerform other duties as assignedEducation and Experience: The requirements listed below are representative of the knowledge, skill, and/or ability required.Required:Certified Nursing Assistant Experience or 10 hours hands on training (can be from previous employer)High School diploma or equivalentMay require the employee to drive on company business. Employee must have a valid driver’s license and meet organization’s driving requirementsProficiency with cellular phones and ability to learn company systems, including web based payroll system Preferred:Ability to work night and weekend hours as well as to be on call1-3 years’ work experience in service industryEssential Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactory.
