Job Title: RN Home Health Profession: RN Home Health Specialty: RN Home Health Duration: 13 weeks Shift: 08:00 AM - 04:30 PM Hours per Shift: 8 Experience: Minimum 1 year of Certified Home Care experience License: Active RN License Certifications: None specified Must-Have: 1 year of Case Management Experience Description: All applicants must have 1 year of Certified Home Care experience. 1 year of Case Management Experience is required. Ability to travel to cases as assigned is necessary. Weekend rotations are once every 6 weeks. As a Home Health RN, responsibilities include assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families. Patient care is delivered within organizational philosophy, policy, and standards of community health practice. Required to perform admission processes and case management responsibilities in accordance with policies and standards of practice. Utilizing thorough and timely electronic documentation and interdisciplinary communications is essential. Experience in hospice, private care, or working as a Case Manager on its own does not meet the qualifications for Certified Home Health Care Management experience. Only roles specifically within a certified home health agency will be considered relevant for the 1-year requirement. Unit covers areas including but not limited to Dorchester, Jamaica Plain, Mission Hill, and Roxbury.
