Dependable Hospice is seeking a compassionate PRN Hospice Chaplain to join our growing Phoenix team.

In this meaningful role, you will provide spiritual and emotional support to hospice patients and their families during the end-of-life journey. We are looking for someone who can meet patients where they are, honoring their individual beliefs, values, cultures, and traditions while providing comfort, presence, and support.

What You’ll Do

• Provide spiritual and emotional support to hospice patients and their families

• Complete spiritual assessments and contribute to individualized plans of care

• Provide prayer, counseling, supportive presence, and other spiritual care based on patient and family preferences

• Support patients and loved ones through end-of-life transitions, grief, and bereavement

• Collaborate with nurses, social workers, physicians, CNAs, and other members of the hospice interdisciplinary team

• Participate in interdisciplinary group (IDG) meetings as needed

• Maintain timely and accurate documentation of visits and services

• Coordinate with community clergy and other spiritual resources when appropriate

What We’re Looking For

• Education or training in theology, ministry, pastoral care, chaplaincy, or a related field

• Hospice, healthcare, or end-of-life spiritual care experience preferred

• Compassionate and respectful approach to individuals of all faiths, beliefs, and backgrounds

• Strong communication and interpersonal skills

• Ability to provide calm, supportive care during sensitive and emotional situations

• Reliable transportation and willingness to travel to patients throughout the Phoenix area

• Dependable, professional, and able to work independently in the field
