McGregor PACE is a Program of All-Inclusive Care for the Elderly (PACE) that supports eligible older adults in Cuyahoga County with comprehensive health and social services. The organization provides in-home health care and adult day programs as an alternative to traditional nursing home placement. Interdisciplinary care teams coordinate medical, nutritional, and supportive services to help participants maintain independence and quality of life. Role Description

The Registered Dietitian is a full-time, on-site role based in Brooklyn, OH, supporting participants in the McGregor PACE program. This role conducts comprehensive nutritional assessments, develops and updates individualized meal and nutrition care plans, and monitors participants’ progress in collaboration with the interdisciplinary team. Daily responsibilities include counseling participants and caregivers on dietary needs, coordinating with nursing and medical staff regarding special diets, and ensuring menus meet regulatory and clinical standards. The Registered Dietitian also participates in care conferences, documents all services in the electronic health record, provides nutrition-related education to staff, and supports quality improvement initiatives related to food and nutrition services.

Qualifications

• Current Registered Dietitian (RD/RDN) credential and licensure (or eligibility for licensure) in the state of Ohio.
• Experience in clinical nutrition, preferably working with older adults, long-term care, or community-based health programs.
• Strong skills in nutritional assessment, care planning, and monitoring of chronic conditions such as diabetes, cardiovascular disease, and renal disease.
• Ability to provide clear, compassionate nutrition counseling and education to participants, caregivers, and interdisciplinary team members.
• Proficiency in documenting care in electronic health records and maintaining compliance with relevant regulations and professional standards.
• Effective communication, collaboration, and time-management skills, with the ability to work in a fast-paced, team-centered environment.
• Bachelor’s degree (or higher) in Nutrition, Dietetics, or a related field from an accredited program; completion of an accredited dietetic internship.
• Knowledge of PACE programs, geriatric care, or community health services is beneficial.
