RN Case Manager: Discharge & Care Coordination

Ochsner Health

Ochsner Health in New Orleans seeks an experienced registered nurse to coordinate discharge planning and utilization review during hospitalization. You will review patient needs, recommend appropriate levels of care, and collaborate with ED physicians, hospitalists and the care team to develop individualized discharge plans.
