## Community Support Worker (CSW) –

CareNationDC is seeking a dedicated **Community Support Worker (CSW)** to provide field-based care coordination, advocacy, and skill-building support to adults with serious mental illness and disabilities in Washington, DC.

The CSW will spend a fair amount of their time in the field, meeting clients in homes, shelters, public spaces, and community settings. This role supports clients in achieving recovery goals, improving daily living skills, accessing community resources, maintaining housing stability, participating in treatment, and working toward employment, education, and independent living.

### Key Responsibilities

• Provide communitybased support and care coordination to assigned clients.
• Assist clients with independent living skills, appointments, benefits, housing, transportation, employment, and education goals.
• Advocate for clients while encouraging selfadvocacy and personal responsibility.
• Link clients to behavioral health, medical, social service, housing, and community resources.
• Monitor client progress, safety, and changes in behavior or functioning.
• Support crisis prevention by identifying concerns and reporting them to the clinical team.
• Complete accurate and timely documentation in accordance with agency, Medicaid, and DBH requirements.
• Collaborate with clinicians, supervisors, RSS staff, outreach staff, and external providers.

### Qualifications

• High school diploma or equivalent required; associate’s or bachelor’s degree in human services, psychology, social work, counseling, or related field preferred.
• Experience working with adults with serious mental illness, disabilities, substance use concerns, homelessness, or community instability preferred.
• Strong communication, documentation, and timemanagement skills.
• Ability to work independently in the field and maintain professional boundaries.
• Knowledge of HIPAA, confidentiality, and client rights.
• Reliable transportation and ability to travel throughout Washington, DC preferred.
• CPR/First Aid certification preferred or ability to obtain after hire.
