Job Description
Title: Physical Therapist

Schedule:
- 40 hours a week
- Shifts vary: M-F 7 am - 6 pm & Saturday 8 am - 1 pm

Assignment Details:
- Contract Length: 13 weeks
- Guaranteed Hours: 40

Requirements:
- Active license as a Physical Therapist
- Current Basic Life Support certification
- Minimum 1 year experience in adult outpatient rehab
- Must have flu vaccinations

Roles & Responsibilities:
- Conduct physical therapy evaluations and develop treatment plans to enhance patients' functional abilities.
- Implement, supervise, and modify therapy plans based on patient assessments and responses to treatment.
- Collaborate with healthcare team members to ensure optimal patient care and prepare for discharge.
- Provide education to patients and their families regarding home exercise programs and self-care methods.
- Maintain clinical records, document patient encounters, and adhere to regulatory requirements and standards.
