Registered Nurse (RN) - Home Health Case Management

We are seeking a Registered Nurse (RN) Home Health Case Manager to provide compassionate, patient-centered care in the community. Responsibilities include: managing a personalized caseload, supervising LPN visits, conducting routine care, evaluations, and start of care visits, and educating patients on conditions such as CHF, COPD, Diabetes, Wound Care, and Medication Management. Collaborate with physicians to obtain orders and develop diagnosis-driven care plans, working closely with therapy teams including PT, OT, and Speech. Qualifications and skills: active Kansas or Compact RN license, valid Missouri Driver's License, strong clinical assessment skills, organized, self-directed, and compassionate communication with patients, families, and healthcare providers.
